For Individual, Partnership, and Corporation organizations –
1. SEC/DTI Registration Certificate
2. BIR Form 2303
3. Municipal Permit
4. Barangay Permit
5. Company Profile
6. BIR Registered Official Receipt (scanned copy of sample)
7. BIR Registered Sales/Service Invoice (scanned copy of sample)
8. Bank Certificate (within the past 3 months)
9. Company ID (scanned copy of sample)
For non-organization Individual service provider –
1. NSO/Municipal CTC Birth Certificate (scanned copy)
2. NBI Clearance
3. Police Clearance
